Personal Data Sheet: an insert you will fill out detailing vital information (name, doctors name, emergency contacts, medical data, blood type, medications, allergies, hospital preference, advance directives, and people to comtact in case of emergency)

Red plastic magnetic envelope: Placed on the refrigerator, which contains the completed Personal Data Sheet

Pocket Size Emergency Information Card  to be carried with a person if outside of their home.

Red Sticker marked "Emergency Information Is On Refrigerator"  placed at the front door, which then directs emergency personnel to the completed data sheet.

The S.A.L.T. Council is a non-profit organization that provides the "EMERGENCY FILE" at no charge to seniors in our community.
